Recall of two drugs in Canada due to dosing instructions errors

The Canadian Ministry of Health announced the recall of two batches of medications sold in the country due to errors in the dosage instructions on their packaging, which may lead to incorrect use posing a risk to patient safety.

The first medication is “Dom-Metformin” at a concentration of 500 mg, produced by Dominion Pharmacal, and is used to treat type 2 diabetes. It has been found that the instructions on the product's packaging do not clearly specify the number of tablets to be taken daily, which may result in an inappropriate dosage.

The second medication is “PMS-Hydromorphone” at a concentration of 1 mg/ml, which is a strong pain reliever available by prescription only, produced by Pharmascience Inc. The issue included inaccurate information on the bottle label, which may lead to administering an overdose of the opioid to the patient.

The Canadian Ministry of Health urged patients to stop using these medications if they have the affected packages, and to contact their pharmacist or doctor for safe alternatives. They emphasized the importance of reporting any side effects or health issues resulting from the use of these medications through their reporting system.

The ministry confirmed that it continues to monitor the situation and is working with the manufacturing companies to ensure that appropriate corrective actions are taken, adding that patient safety is among its top priorities.
