The Transportation Safety Board of Canada said it found several safety shortcomings during its investigations into the Toronto ferry collision that resulted in 12 people being injured two years ago.
The investigation, which was released on Monday, found that the City of Toronto did not have written procedures to determine a safe approach speed while docking.
Instead, decisions regarding docking speed were left to the discretion of the ship's captain, the report says.
The report also found that the vessel had only six crew members on board, which is fewer than the 13 crew members recommended by Transport Canada.
The organization makes several key recommendations based on its investigations, including the implementation of written docking procedures.
The Sam McBride ferry struck the dock as the vessel approached the Jack Layton ferry terminal on August 20, 2022, resulting in 12 injuries.
